Email Waiver

By signing my name and writing my email address below (or transmitting this completed form electronically), I hereby give permission to the Our Lady of Hope Regional School Athletic Association (“OLHAA”) and all of the OLHAA Board Members and Officers, to send me periodic and unannounced emails regarding news, events, updates, registrations, fundraisers, and any other matters related to OLHAA’s sports and/or programs. I also understand that by signing or transmitting this form to OLHAA electronically (with or without my signature) I am indicating my agreement to these terms. I understand that OLHAA may form an email group, in which my email address may be included, for the purposes of keeping members of the community informed about OLHAA sports and/or programs.

I understand that I may request that OLHAA remove my email address from the email group and/or cease any further correspondence via email, by sending a request via e-mail to the webmaster at the ourladyofhopeaa.org website or by making a written request to the Athletic Director via the Our Lady of Hope Regional School office.

Note: Our Lady of Hope Athletic Association will not sell or distribute your email address to any third party vendors or solicitors without your consent.
